Consultation request privacy notice
This notice covers the consultation form on the Okade website. It is separate from the handling of patient data within the clinic platform.
Information you provide
Required fields are your clinic name, contact name, work email, role, team size and consent to consultation contact. Phone, plan of interest and your message are optional. Do not submit patient details or medical records.
Purpose and delivery
The Okade team uses these details to respond to your demo and onboarding enquiry. Your submission is delivered through an email provider to the team’s configured inbox. This website does not save consultation submissions in a separate database.
Retention and deletion requests
Delivered enquiries are managed in the team’s email system. Reply to the team’s email to request access, correction, deletion or an end to consultation contact. Ask the team about its applicable retention period and email processing arrangements during your consultation.
Your choice
Submitting the form is optional. If you do not consent to the use of the required details, you cannot request a consultation through this form. Consent covers responding to your enquiry and is not a separate subscription to marketing emails.
Website usage
No advertising trackers are installed by default. To reduce spam, request limits may temporarily use a hash of your email address and, depending on server configuration, your IP address. Hosting and email providers may process technical logs needed to operate their services.
